Background Check Permission Form
Here at the United Methodist Church of Monroe child safety is paramount. In our continued efforts to make our worship and Sunday school a safe place for children, the Church Council on October 28, 2008 adopted a formal policy in relation to “safe sanctuaries.”
We are currently working toward compliance that will bring us to an equal stance with surrounding churches and denominations. The first step toward compliance is to have every person who is going to be working with youth complete a background check. We need everyone to fill out the form referenced below and submit it to the church office.
Please protect yourself and do not email this form to the church office: snail mail or direct drop off are the only options for turning in this form.
The average cost per person for this service is $10.00. If you can afford to make out a check to the church to offset this expense please do so. Unless authorized by you, every report will be seen only by the pastor only and stored in a locked filing drawer in the pastor’s office. Individuals interested in accessing their own file may do so by appointment with the pastor.
